Welcome to the West Salem School District

The West Salem School District is now offering central registration at the District Office located at 405 Hamlin Street East.  Registration hours are Monday through Friday from 8:00 am to 4:00 pm. 

For your convenience here are the required enrollment forms.

In addition to the completed enrollment forms, you will also need to provide the following information:

     1.  Proof of residency, which may be one of the following:

                   *Current lease/rental agreement

                   *Current utility bill

                    *Home purchase agreement

     2.   For 4K and kindergarten students, your child's birth certificate
