The School District of West Salem offers central registration and enrollment at the District Office located at 405 Hamlin Street East. Registration hours are Monday through Friday from 8:00 am to 4:00 pm.

New Family Enrollment Form
Proof of Residency, which may include one of the following
- Current lease/rental agreement
- Current utility bill
- Home purchase agreement
For 4K and new Kindergarten students, the original Birth Certificate.
